We are looking for a dedicated and experienced Clinical Case Manager to join our growing, client-focused team at Rehab for Independence. This is a rewarding and varied role where you will play a central part in each client’s rehabilitation journey—coordinating care, setting clear and achievable goals, and empowering clients to regain independence and quality of life. You’ll work closely with clients, families, and multidisciplinary professionals to ensure holistic, tailored support that evolves with the client’s progress.
Our clients often present with a range of physical, cognitive, sensory, and emotional needs following life-changing injuries or diagnoses. Your clinical background in Occupational Therapy, Physiotherapy, Psychology or Nursing will enable you to assess needs, implement meaningful interventions, and advocate for the client’s best interests throughout their rehabilitation journey.

Essential Skills& Attributes:
• HCPC registration and recognised Occupational Therapy qualification
• Strong background in neuro rehabilitation
• Excellent communication and interpersonal skills
• Able to manage your own caseload and work flexibly
• A positive, solution-focused approach with a genuine passion for helping others
Desirable Experience:
• Experience working within the medico-legal sector
• Familiarity with case coordination and multi-agency working
