Clinical Case Manager

About the role

We are seeking a Clinical Case Manager to join our dedicated team supporting clients across the North West of England. This is a fulfilling and dynamic role where you will use your clinical expertise to make a meaningful difference in the lives of individuals recovering from life-changing injuries and conditions. You will have a background in Occupational Therapy, Physiotherapy, Psychology or Nursing, enabling you to deliver outstanding care coordination and client-centred rehabilitation planning. Working closely with clients, families, and multidisciplinary teams, you’ll be instrumental in supporting our clients to achieve their goals and maximise their independence.

We are looking for a dedicated and experienced Clinical Case Manager to join our growing, client-focused team at Rehab for Independence. This is an exciting opportunity to be part of a dynamic private healthcare service that specialises in supporting individuals with acquired brain injuries, neurological conditions, sensory needs, and other complex health challenges.

This is a rewarding and varied role where you will play a central part in each client’s rehabilitation journey—coordinating care, setting clear and achievable goals, and empowering clients to regain independence and quality of life. You’ll work closely with clients, families, and multidisciplinary professionals to ensure holistic, tailored support that evolves with the client’s progress.

Our clients often present with a range of physical, cognitive, sensory, and emotional needs following life-changing injuries or diagnoses. Your clinical background in Occupational Therapy, Physiotherapy, Psychology or Nursing will enable you to assess needs, implement meaningful interventions, and advocate for the client’s best interests throughout their rehabilitation journey.

About You

 

Essential Skills& Attributes:

• HCPC registration and recognised Occupational Therapy qualification
• Strong background in neuro rehabilitation
• Excellent communication and interpersonal skills
• Able to manage your own caseload and work flexibly
• A positive, solution-focused approach with a genuine passion for helping others

Desirable Experience:

• Experience working within the medico-legal sector
• Familiarity with case coordination and multi-agency working

£35 - £45k per year dependant on experience
