About the role

We are seeking a motivated and compassionate Rehabilitation Facilitator to join our dedicated neurorehabilitation team, supporting clients across the Northwest of England. Thisis a rewarding and hands-on role, supporting individuals living with the effectsof life-changing neurological injuries and conditions to maximise independence and quality of life.

Working under the guidance of qualified therapists and as part of a multidisciplinary team, you will support the delivery of client-centred rehabilitation programmes within clients’  community environments. About the Role

We are looking for a Rehabilitation Facilitator to support adults and children with complex neurological conditions within our growing private neuro rehabilitation service. This is a practical, client-facing role, working closely with Occupational Therapists, Physiotherapists, Speech and Language Therapists, and Case Managers.

You willassist with the implementation of therapy programmes, supporting clients todevelop functional skills, increase independence, and achieve their personalrehabilitation goals.

Key Responsibilities

• Support the delivery ofrehabilitation programmes designed by qualified therapists
• Work directly with adults and children with neurological conditions in homeand community settings
• Assist clients with functional activities, daily living skills, and therapeutic exercises
• Support cognitive, physical, and behavioural rehabilitation programmes as directed
• Encourage client engagement, motivation, and participation in meaningful activities
• Record progress accurately and provide feedback to the wider MDT
• Maintain professional boundaries and uphold organisational policies and standards

Person Specification

Essential:
• Experience working in a health, social care, or rehabilitation setting
• A compassionate, reliable, and professional approach
• Confidence working independently in the community with appropriate support
• Strong communication and organisational skills
• Willingness to learn and engage in training and supervision
• Full UK driving licence and access to a vehicle

 

Desirable:
• Experience supporting individuals with neurological conditions
• NVQ Level 3 (or equivalent) in Health & Social Care or Rehabilitation
• Experience working within private practice or community-based services
